The following information is needed to process the admission:


Patient's name, address, telephone, date of birth, social security number and emergency contact.


Name, address and telephone of the patient's insurance carrier; insurance policy group and contact number.


Insured's employer company name, address and telephone. Name, address, and telephone of insured if other than the patient's relationship to the insured.
Insurance Accounts
The insurance company will be contacted for the benefit verification. The patient or responsible party will then be advised, during the intake process, of any deposit or co-payment required and not covered by insurance.

Cash Accounts
Cash accounts and payment terms are based upon the length of stay and are scheduled with the patient or responsible party prior to admission. Visa, Discover, MasterCard and American Express are accepted.

At The Meadows, our main concern is providing recovery and a more fulfilling life for those who seek our services. Where insurance coverage does not apply, we suggest that patients talk with our business staff. Our Intake/Financial Coordinators will assist with a review of costs and payment options.
